I have signed up for "Internet Call Manager", offered by my local phone company (Verizon). This is software somewhat similar to Callwave's Internet Answering Machine but which allows you to:
1) disconnect from the internet and answer an incoming call, or
2) send the caller a message saying that you will call right back, or
3) send the caller a message asking that he call you back later, or
4) forward your caller to an alternate number, or
5) ignore the call entirely (your caller will hear your phone continue to ring.)
Now, I haven't received a call yet so I can't swear that the thing works but it seems like a good idea and the price is reasonable (~$6/month) with no need to have "call-forwarding on busy" enabled. That makes it approximately half the price of Callwave which (last time I used it) cost $6/month plus the added fee for "call forwarding on busy".
Might be worth looking into (through your local phone company) for those with a dial-up connection.
